Life Story Supplemental Benefit Enhances Patient-Centered Care

MemoryWell™ life stories were approved for 2023 as a supplemental benefit for a Medicare Advantage Organization participating in the VBID hospice carve-in demonstration project.


Life stories are offered to plan members who choose an in-network hospice provider. The stories are based on a one-time interview with a MemoryWell writer and shared with care teams to promote care coordination and person-centered care.

Early feedback shows high satisfaction among clinicians, patients and family caregivers for the new benefit.


In early survey results, 100% of family members agreed that the Life Story was beneficial to their family member and agreed that the Life Story improved their relationship with the care team. Among clinicians, 100% of those surveyed agreed that the Life Story helped them provide care based on the patient’s unique needs and wishes.
